Notes :-
Osteoarthritis (OA) is a disease that affects knee joint most commonly, as the age progresses after fourth decade of life. The surface within joints become damaged so the joint doesn’t move as smoothly as it should. OA is usually diagnosed based on clinical signs and symptoms when examining joints.
Occurring most frequent in the knee, the prevalence of OA in this joint increases with age from approximately 1% in people aged 25-34 years to >30% in people aged > 75years. NSAID’s are commonly used for the management of OA and are effective in relieving the pain and physical dysfunction. NSAID’s are used to treat pain, swelling and inflammation from medical conditions such as OA.
NSAID’s may increase the chance of heart attack or stroke that can lead to death. This chance increases with longer use of NSAID’s in people who have heart disease. However , their use is associated with GI effects, ranging from mild heartburn and bloating to serious ulceration, perforation, obstruction, and bleeding, and with hepatic and renal toxic effects.
Subjects with OA whose symptoms are not adequately controlled with conventional or oral NSAID’s may require topical NSAID’s. International guidelines on OA care recommend the use of topical NSAID’s as an alternative to oral NSAID’s. Also , the OA research society international guidelines recommended topical NSAID’s as a first line therapy for subjects with OA and it was observed that the topical NSAID’s had an equivalent efficacy and a lower frequency of adverse events compared with oral NSAID’s.
Diclofenac is a NSAID, which works by reducing hormones that cause inflammation and pain in the body. The study medication in the present study is a gel formulation that contains the diclofenac sodium as its active ingredient. Various studies have reported diclfenac sodium gel to be safe and effective in subjects of OA of the knee.
In our department we had conducted drug trial under the heading “A Prospective , Multi-Center, Randomized, Double-Blind, Vehicle-Controlled, Parallel-Group Bioequuivalance Study With Clinical Endpoints Comparing Generic Diclofenac Sodium Topical Gel 1% of Cipla Ltd., India To Voltaren Gel (A Diclofenac Sodium Topical Gel 1%) Of Endo Pharmaceutical Inc., USA In Subjects With Osteoarthritis Of The Knee” from March 2016 To February 2017, under the Guidance of Dr. Sanjay Patil (Head of the department in orthopaedics, Bharati Hospital) and Dr. Gopal Pundkare (Assistant Professor in department of orthopaedics, Bharati Hospital)
Innovation is the need of the day. The field of Medicine and medical technology, especially, is always on the search for better treatment facilities and drugs to combat the growing ill health of the society and modern living. It is a matter of great pride for all of us when an Indian comes up with an invention that can combat these problems and make lives of patients easier. Dr. Arvind Bhave did exactly that by developing special needles for spine surgeries.
Dr. Arvind Bhave is the founder of a spine clinic, Bhave Hospital in Pune. After completing his MBBS and MS- Orthopedics from BJ Medical College, Pune, he went on to do his fellowship training from prestigious institutes in Israel, Japan and Korea and visited many spinal and neuro-spinal training institutes across the world.
When Dr Bhave came back to India, he found that the medical instruments and gadgets he had imported were unaffordable to patients here. Moreover, problems like osteoporosis and related spinal fractures were growing among senior citizens who also suffer from other problems of high blood pressure, diabetes etc. Dr. Bhave realized that instead of big open surgeries, the minimally invasive surgeries would be beneficial to the Indian population. These are under local anesthesia and hence, risk is reduced. Also, a day’s hospitalization is enough. No stitches are required and just bandaged dressings are enough.Hence, these surgeries would be the future for Indians. However, the high cost of imported equipment was a big hurdle. Not to give up, this doctor decided to come up with Indian equipment, which would be economical and effective both in function and utility. Thus, he developed unique low-cost needles for his patients.
Dr. Bhave says about his invention, “The needle device is patented under the IP Act in 2008, trade marked and the procedure using this device is copyrighted as ‘AB needle for Vertebroplasty’. It is used for the surgical treatment called, ‘Vertebroplasty’. This is a minimally invasive surgery for fixing fractures of spine due to osteoporosis or cancers spread in spine. We usually pass cement through this device and it gives strength to the affected vertebra(spine). This is done fully under local anesthesia and the patient goes home with a small bandage dressing. Other uses of this device are to take biopsies from vertebral body or any other bones for further diagnosis and treatment.”
The AB Needle stands out from the rest; its main features are: a] safety,b] locking system,c] multiple channels which allow 360 degrees of complete, uniform cementing of the affected vertebral body.
This innovation boasts of better safety margins and has a locking system to reduce failure or cement blockage during surgery. It has multiple channels, which allow complete 360 degrees discharge of cementing material.
And probably the best part of the AB Needle is its price; Says Dr. Bhave, “It is cost effective. Compared to 3000 USD of imported devices, the AB Needle is just 1700 Indian Rupees, thus bringing down the cost of surgery and making it more affordable to common Indian population -The Aam Aadmi!” So, how can we avail of the AB Needles ? Dr. Bhave reveals that efforts are on to market it through various surgical companies in India and abroad. As of now, they can be directly collected from him.
Dr. Bhave has personally used the AB Needles in more than 500 surgeries and has done demonstration surgeries and lectures in India and in countries like Japan, Germany, Austria and many more. The needle has been used by many spinal surgeons in many states across the corners of India from Gujarat to Assam.
The AB Needle which comes as a boon to many osteoporotic patients has given Dr. Bhave some memorable cases. He recalls a one 67 yrs mango-plucker from Konkan, who had a fall in April, 2013. His spine fracture prevented him from earning his living. A week after surgery, he was back to pluck mangoes in May, the season of Alfonso mangoes in Ratnangiri,Konkan.
There was another incident where a 65year old diabetic and asthmatic lady was bedridden for 6 months after suffering a spinal fracture during a bus travel.Dr Bhave happily says, “5 months after a successful Vertebroplasty, she did a Char Dham Yatra and brought me ‘Prasad’!

Medical Colleges are the places where learning is sacred. Learning also means growth. For growth in any field, research is a must. Who are better suited for research than professors with years of practical experience and eager students wanting to learn everything? This combination helps Medical Colleges ensure that they stay at the cutting edge of advancements in the medical field and at the same time contribute to this advancement by doing original research.
The importance research activity was identified by the Bharati Vidyapeeth Medical College since its establishment in the year 1989. Since then BVUMC has given priority to improve research related activities within the campus. It is for this reason that an independent research unit IRSHA – Interactive Research School in Health Affairs was established. IRSHA has been specially established for promotion, development and coordination of various research activities. Main agenda of IRSHA is to collaborate research of various disciplines within BVUMC as well as with external research institute like NCL and other international universities.
BVUMC has provided an excellent platform not only for UG and PG students but also to its faculty members to participate in research activities. UG students are assigned projects in small groups by department of Community Medicine. PG students are given broad areas of topic for research which helps to create and promote research aptitude. Faculty members of various departments are encouraged to present scientific research papers at various national and international conferences.
The research society of BVUMC is authorized for funding important research projects. Similarly the research committee at the university level is also entitled for funding various research projects.
The Research Society of BVUMC was established to encourage the idea of research among the faculties of all the disciplines and the students to undertake more research projects.
Teachers are given special study leave for research purpose along with seed money for their project from B.V. University. Students are given financial support in the form of ICMR scholarship along with all possible help to use college and hospital resources required to carry out the project.
There have been a number of publications at both International and National level from the college staff. A number of staff members and students have presented their work in various national and international conferences. Many of the faculty members are invited as guest speakers and to deliver key note address at the major knowledge events. The college has received more than Rs. five crores in grants from various national and international agencies for carrying out research.
Medical course is a long course of four and half years and students very often tend to get short of confidence, insecure, depressed, especially those students coming from various corners of India and from abroad.
Facilities are provided to the students for their academic progress as well as extracurricular development.
When the college opens for the first year students, the Medical Education Unit of the college conducts a three-day orientation programme. Students are counseled regarding medical curriculum, pattern of teaching and learning, study skills, stress and time management. Later, when the students are exposed to clinical teaching, orientation is conducted regarding development of clinical skills.
The academic in charge of pre-clinical, para-clinical and clinical batches receive inputs about performance and attendance of the students. Parents of those students with unsatisfactory performance are informed regarding their ward’s progress. Counseling sessions are held between heads of department and such parents and students to sort out academic and even non-academic problems of these students.
The student’s avail of all other facilities such as library, digital library, internet, well equipped lecture halls with LCD projection.
Several cells like counseling cell, guidance cell, grievance cell exist for the students. Facilities are provided for students to participate in sports and cultural activities. Students are encouraged to participate actively in sports activities, cultural festivals and various community outreach programmes. Every year the students bring out an issue of college magazine “The Elixir” and showcase their literary skills.
It is our endeavor to carve out an all round, humane, competent physician out of each and every medical student.
The Medical College considers its “Staff” as its most important asset and strives for its well-being. There are several schemes devised and implemented for the welfare of the staff members.
The Bharati Vidyapeeth Health Scheme (B.V.H.S.) ensures that all the staff members (Teaching and Non-Teaching) can avail the investigative, diagnostic and treatment facilities; howsoever costly they may be, at Bharati Hospital. A nominal amount is deducted from the salary for this purpose and all the above facilities are given free of cost.
The Sevak Kalyan Nidhi (S.K.N.) is another such welfare scheme implemented specially for the non-teaching staff. It helps them in their needs such as education of their children, housing loans etc.
Computers and Internet have become the most vital and essential part of day-to-day functioning. A fast speed Internet facility is provided in every department and a multi-terminal digital library is set up in the college building. The staff can access almost all indexed journals and various textbooks and they need not spend a large amount for subscription to these journals. This helps them to update their knowledge, send their research articles and also find references. The digital library staff is available full time for any assistance.
Research and up gradation of knowledge is a priority in the college. Funding is given to the staff members to attend conferences, workshops and training programmes. The staff is encouraged to actively participate in them, present their work and have interaction with authorities in their respective subjects. The college organizes several surgical workshops every year wherein reputed surgeons from India and abroad train the faculty members and improve their surgical skills. A state of art research facility – IRSHA – exists in the college campus and every help is given to each and every staff member desirous of carrying out research.
Besides the curricular activities, the staff meets informally at various get together events organized by the college on the occasion of research society conference or university foundation day.
